    Questionnaire as to whether a person has limited capability for work.

    A friend of mine has just rung me and tol me that she has just received a form titled

    "Questionnaire as to whether a person has limited capability for work."

    She says her doctors notes are written inside it and her doctors signature is on the forms. I am baffled by this. I can't get to her house until tomorrow so does anyone know what this form is? If it is a new ESA50 why are comments from her doctor in there?

    Is your friend applying for ESA/appealing ESA/wanting to be assessed for a different group?

    Has your friend received any help from a welfare advisor/CAB?

    Some welfare advisors/benefits specialists send these forms to the claimant's doctor/other medical professionals with a list of the descriptors so that the medical professional can write their notes with reference to the ESA descriptors.

    If you google ' ESA appeal adviceguide' you will see that CAB sometimes uses this method to support a claimant. Click on the Leiceister ESA appeal link - it opens in a word document so can't post a link - sorry.

    You can use the form as evidence for claim/appeal etc. It does not replace the ESA50. It is just a form of medical evidence that the claimant can use.

    This sounds like it may be an ESA113, is this 'questionnaire' part of a bigger file of papers like a Tribunal Bundle for instance ??, as you generally don't get to see them unless they're included in something like that.

    To do a link like that to a doc, xls, or similar from google-

    • Right click on the blue heading in google.
    • Select 'Copy Link Location' from the pop up menu.
    • Come back here and right click in the reply box.
    • Paste the link into the reply box.


    Changing the linked text to read "click this for the link" or whatever I will leave as an excercise for now.
