Need some help with the ESA50 form please

Hi,

I wondered if anyone could help me with a concern i have over a question in the ESA50 form that ive been asked to complete and send off. (I currently suffer with anxiety and depression which affects my day to day life quite badly unfortunately at the moment)
The question i am struggling with on the form is the question on page 3 which asks for any times or dates in the next 3 months when i cannot go for a face to face assessment, in a few weeks i have family coming over to stay with me for a couple of days, they travel from quite a distance so i wouldnt be able to attend an assessment on these days, would it be okay to mention this and should i not just give dates but also explain why i wont be free on those dates or would just giving the dates be enough? I'm quite nervous about filling this form in so i would appreciate any advice.

Hi, I've got to fill one of these out to. The question on page two is reasonable. I guess they could send you a letter asking you to attend an assessment but if they didn't know you're in hospital or away they'd wonder why you missed your appointment.

Just say 'you're not available between 6 -10 July (or whatever dates) due to family reasons.

The rest of the form is more important for you. Could I suggest you look at this website and pay the subscription to get inside information about how to fill the form out especially if you have mental impairments as it will give you tips on the best way to fill out the form. I'd also write drafts of your answers on another piece of paper to in case you change your mind on some answers.

If you need extra support to fill this 20 page document ask the Citizens Advice Bureau or see a Social Worker.
