    I've no doubt that there is something in the HCP guidance about what they mean and what should be in there, just a case of tracking it down.

    From what I can find in the assessors admin. guidance those fields should be filled in by the DWP before they get it.
    Getting hold of DWP internal guidance is a bit more tricky.

    So I got the Medical report today.

    Report has errors mentioning walking sticks, says someone present during assessment, correct medication but wrong amounts! I suppose it's better just to let all that go...

    There was no 'Prognosis' or 'Justification of advice' section like on previous reports - just at the back a new? section 'LCW and LCWRA info' where it says 'Review in medium term' and not actually a specific length of time mentioned.

    What I'd like to know is: has the report form format changed? Two previous reports were titled 'ES85A' and are shorter in length, this is titled 'ESA85' and 8 pages. Then again I found another report from way back 'ESA85' 20 pages but that did contain Prognosis info.

    Still waiting for the Decision Maker report (LT54?) Maybe it's the DM who put 18months.

    I wouldn't be surprised if they have canged it again, to take account of the fact that WCAs are now also done for UC as well as ESA.

    The document jacket I linked before used to be called an ESA55, now its a WCA55 because it's also used for UC WCA's.
    (And I would expect that to change shortly as it references 'Live Service UC' which is being phased out).

    As the assessments are identical anyway it makes sense to only have one set of paperwork that covers both benefits.

    Finally got a type of DM Report after second request only this seems to be in a different format now too i.e.. they just cut and pasted the relevant bits into a normal letter format rather than send the actual LT54 form which I've received previously...

    Sounds a bit funny, they'd have to disclose the actual LT54 if you were appealing.

    If you realy want it then you could try using GDPR 'Right of Access' to get them to release it.
